The Office of Transport Safety Investigations conducts investigations into transport safety occurrences on the NSW rail network under Section 67 of the Rail Safety Act 2008.
Under Section 65 of the Rail Safety Act 2008 OTSI may require an accredited operator to inquire into and report back to OTSI on any railway accident or incident that may affect the safe carrying out of railway operations for which the operator is accredited.
