Following the notification of a safety occurrence to the OTSI Duty Officer the rail operator may be requested to complete an investigation into the matter. This request will constitute a requirement by the Chief Investigator under Section 65 of the Rail Safety Act 2008.
This investigation may take the form of a formal operator investigation carried out in a manner consistent with Australian Standard 5022 or a 72 Hour Investigation Report. The requirement to complete either of these investigations will be confirmed by a letter or email to the accredited rail operator.
