What happened
An investigation has commenced into a passenger train overspeed through a set of points between Doonside and Blacktown in Sydney on 21 April 2024.
At approximately 1436, a passenger service travelling between Penrith and Berowra traversed points near Blacktown at a speed of 99 km/h, where the turnout speed was 25 km/h.
The driver was reportedly thrown from their driving seat, but there were no reports of injured passengers. Although there were no reported injuries, an exceedance of this magnitude has the potential for significant consequences.
Investigation scope
The investigation scope includes risk controls for overspeed, condition of rollingstock and track infrastructure, train operations and scheduling, the train crew, communications and gathering any other relevant information.
A final report will be released at the conclusion of the investigation. However, should a critical safety issue be identified during the course of the investigation, OTSI and the ATSB will immediately notify relevant parties, so that appropriate and timely safety action can be taken.
