Following the notification of a bus fire/thermal incident to OTSI, the operator may be requested to complete an investigation into the matter. This information also will help to decide whether an OTSI investigation be carried out.
To assist with the completion of this investigation, a Bus Fire/Thermal Incident Information Collection Form may be used.

To complete the form, you will be asked to provide the following information:
General
- Date, time and location of the incident
- Bus Incident Management Database (BIMD) number
Bus details
- Registration number/depot
- Vehicle type
- Fuel type
Thermal event/fire details
- Fire or thermal
- Level of damage
- Cause of incident
- Part at fault (if known)
- Was on board fire extinguisher used and result
- Did the engine bay fire suppression system work effectively (if activated for this fire)
- Did fire brigade attend, and if so what was the fire brigade action
Administration
- Contact details
- Date report completed
Additional information
As well as this information, please attach any site photos or photos taken during investigation to the report.
If the incident involved a faulty part of brake, please include as many details of faulty part or brake as possible. For example: brand, part number.
